DBQ Evaluations

A Disability Benefit Questionnaire (DBQ) evaluation is a structured psychiatric examination that documents your current mental health symptoms and their functional impact, using the same forms VA examiners use to rate disability claims. This gives your claim clear, standardized clinical documentation.

$850 per evaluation

Nexus Letters

A Nexus Letter is a written medical opinion connecting a current diagnosis to an in-service event, injury, or exposure using "at least as likely as not" language. This is often the missing piece the VA needs to service-connect a condition.

$650 per letter

Prepare for Your Appointment

  • Complete brief forms.
  • Upload supporting documents: DD214, military treatment records, applicable civilian treatment records, current disability ratings (if applicable), and previous VA claim denial letter (if applicable).

If you're working with a coach, attorney, or VSO, they can help you with this.

Evaluation Appointment

  • DBQ appointments: 90 minutes
  • Nexus appointments: 60 minutes

Receive Your Reports

You'll receive an initial draft to review. Once you approve the draft, the final report will be sent to you.
